Looking for an outline of an Occupational Health and Safety Program? 

Here's an OH&S Program "shell".  Once customized for your organization, it should detail all elements of your Safety Program. It includes helpful templates for Incident/Accident Investigations, Inspections, etc.

The BCMSA and WorkSafeBC (then WCB) entered into a Joint Compliance Project, in the mid-1990's, to develop Supplemental Safety Programs (SSP's) to help employers comply with the newly revised Occupational Health and Safety Regulation.  These SSP's include program documentation and PowerPoint training programs.

Please be advised that these have not been updated since 2003; if you utilize them within your organization, you will need to ensure the Regulations they reference are accurate.
